If your are within the La Puente Valley Water District (LPVCWD) coverage area, please read the below information on starting water service.

For Owners:

• Verification of Ownership & Warranty of Payment Form must be completed.

• Proof of ownership: A copy of Escrow Papers or Property Tax bill with owners name and address on the document.

• Copy of your current Driver’s License or Identification Card, Social Security Number and/or Tax ID Number.

• $25.00 Non-Refundable Application/Transfer Fee.

For Tenants:

• Tenant application Form must be completed.

• Rental or Lease Agreement.

• Copy of your current Driver’s License or Identification Card, Social Security Number and/or Tax ID Number.
